Rhinoplasty: What to Expect Before, During, and After Surgery

What Is Rhinoplasty?

Rhinoplasty — commonly called a nose job — is one of the most technically demanding and rewarding procedures in plastic surgery. It involves reshaping the bone, cartilage, and soft tissue of the nose to improve its appearance, proportion, or function. When performed with precision and an understanding of facial harmony, the results are transformative yet unmistakably natural.

The nose sits at the centre of the face. Even subtle changes in its projection, width, or tip definition can dramatically improve overall facial balance. That is why rhinoplasty demands both surgical expertise and an artistic eye.

Am I a Good Candidate?

Rhinoplasty is suitable for adults whose facial skeleton has finished growing — typically after the age of 18. Good candidates are individuals who:

  • Are bothered by the size, shape, or proportion of their nose relative to their face
  • Have a nasal hump, a drooping tip, wide nostrils, or asymmetry they wish to correct
  • Have difficulty breathing due to a deviated septum (functional rhinoplasty)
  • Are in good general health and have realistic expectations
  • The most important factor is motivation. Rhinoplasty should be done for yourself — not to meet someone else's expectations.

    The Consultation

    Your first consultation is a detailed, unhurried conversation about your goals, concerns, and anatomy. During this visit:

  • Your surgeon will examine your nose from multiple angles and assess the thickness of your skin, the strength of your cartilage, and the relationship between your nose and other facial features
  • Digital imaging or 3D planning may be used to help you visualise potential outcomes
  • Your medical history, medications, and any previous nasal procedures will be reviewed
  • The surgical approach — open or closed — will be discussed based on the complexity of your case
  • There are no standard noses in rhinoplasty. Every plan is designed around your unique anatomy and aesthetic goals.

    Open vs Closed Rhinoplasty

    Closed (endonasal) rhinoplasty involves all incisions placed inside the nostrils. There is no external scar, and swelling typically resolves faster. It is well suited for patients who need more limited corrections.

    Open rhinoplasty involves a small incision across the columella (the strip of tissue between the nostrils) in addition to internal incisions. This gives the surgeon full visibility of the nasal framework and is preferred for more complex reshaping — particularly of the nasal tip. The columellar scar, when properly placed and sutured, becomes virtually invisible within a few months.

    The Procedure

    Rhinoplasty is performed under general anaesthesia and typically takes between 1.5 to 3 hours depending on complexity. During the procedure, the surgeon may:

  • Remove or reshape nasal bone and cartilage to reduce a hump
  • Refine and reposition the nasal tip
  • Narrow the nostrils
  • Straighten a deviated septum
  • Use cartilage grafts (often from the ear or rib) to add structural support or projection
  • You will not feel any discomfort during surgery. Most patients go home the same day or after a brief overnight stay.

    Recovery: Week by Week

    Days 1–3: You will have a nasal splint and internal packing if used. Swelling and bruising around the eyes is normal and peaks at 48–72 hours. Keep your head elevated and avoid blowing your nose.

    Days 5–7: The external splint is removed. Much of the initial bruising begins to fade. You will be able to see the rough new shape of your nose, though significant swelling remains.

    Weeks 2–3: Most visible bruising resolves. You can return to light work and social activities. Avoid glasses resting on the nose.

    Weeks 4–6: Swelling continues to reduce. You may resume light exercise.

    Months 3–6: The nose looks dramatically more refined. The majority of swelling has resolved and the new shape becomes apparent.

    Month 12+: Final results. Tip swelling, particularly in thicker-skinned patients, may take a full year or more to fully settle.

    Managing Expectations

    Rhinoplasty results are gradual. Patience is essential. The nose you see at 2 weeks is not the nose you will have at 12 months. Rushing to judge the outcome early leads to unnecessary anxiety.

    It is also important to understand that perfect symmetry in the nose — or anywhere on the face — does not exist in nature. The goal is meaningful improvement and harmony, not perfection.

    Risks and Complications

    Like any surgical procedure, rhinoplasty carries risks. These include:

  • Swelling and bruising (expected and temporary)
  • Asymmetry or irregularities requiring revision
  • Breathing difficulties (temporary or, rarely, persistent)
  • Infection or poor wound healing (uncommon)
  • Changes in skin sensation
  • The risk of complications is significantly reduced by choosing a board-certified plastic surgeon with specific expertise in rhinoplasty, following pre- and post-operative instructions carefully, and being a non-smoker.

    Rhinoplasty, when done well, is life-changing. The decision to proceed should be made with full information, no external pressure, and confidence in your surgeon.
